Obtaining Clearances

Potential employees of the Apollo-Ridge School District are required to submit these clearances prior to being recommended for hire. 
 
Employees and volunteers are required to provide school districts with updated clearance certifications (Criminal Record Check, Child Abuse History Certification Mandated Reporter, and FBI Fingerprint Clearance*) every five (5) years. When you submit clearances, please keep a copy for your records, as the Human Resources Office will not be responsible for providing you with copies.  New employees will be asked to register with COMPLY (email invitation will be sent to you) in order to help you keep track of when your clearances are due.
 
Maintaining updated clearances is a condition of employment, and it is up to the employee to be aware of when his/her clearances expire.
Employees must upload all clearances into the Comply system.

*The FBI Fingerprint Clearances is not required for a volunteer as long as the volunteer has been a resident of Pennsylvania for the entirety of the previous ten (10) years and signs an affidavit attesting that they are not disqualified from service based on conviction of offenses that would prohibit such service. 
 
Volunteers may use these same links, however, please choose the VOLUNTEER portals so you will not be charged. Individuals who have lived within the state of Pennsylvania may waive the FBI fingerprint check by completing the waiver form below. See Volunteer links below. 
PA Criminal Record Check (Act 34-- $22.00)
To complete this clearance online, fill out the required information and pay the fee with a credit card.  You will have the results back in a matter of minutes.  Be sure to click on the "CERTIFICATION FORM" link to print the actual certificate that is needed.  Print several copies of the results from the screen or save it as a PDF file to be accessed later because the watermark in the printout will not show in a Xerox copy. If you are unable to complete the clearance online, you may mail the form in but please keep in mind the return will take a few weeks.
 
Federal Criminal History Record Information (Act 114 -- $23.85)
You must first register for an ID before you can get finger-printed at an approved location.

  3. Select Schedule or Manager Appointment
  4. Enter Name, Birthdate, Email and Phone number
  5. Create a Security Question and Answer (Remember for future reference)
  6. Enter Citizenship Information
  7. Enter answers to Personal Questions
  8. Enter Personal Information
  9. Enter Address
  10. Choose Document that you will bring with you for ID
  11. Select Location
  12. Schedule Appointment
  13. Submit Registration
  14. Print Confirmation
  15. Payment is processed when prints are taken
  16. Provide UEID to employer once prints have been processed
PA Department of Human Services (DHS) Child Abuse History Certification (Act 151 -- $13.00)

You many complete the application (follow directions and remember your log-in credentials) and pay online with a credit card. You will receive an email when your clearance has been processed.  Log back into your account to print the certificate.  If you send the paper application, you must pay with a money order.  Results may take 3-5 weeks to be returned, so please allow enough time!  Make sure you provide the CERTIFICATE, not just the receipt.

 
Arrest or Conviction Form (Act 24 -- no cost)

Please print and read the following form carefully, complete it, and return to the Human Resources Office.  

Commonwealth of Pennsylvania Sexual Misconduct/Abuse Disclosure Release (Act 168 -- no cost)
This is for new employees only (not required for volunteers or existing employees). Please note: You may need to complete multiple forms, depending on your employment history.  Fill out a form for 1) your current employer, no matter what type of entity it is, 2) any school entity(ies) where you were previously employed in any capacity and 3) any other employer(s) where you had direct contact with children.  Return the completed form(s) to the Human Resources Office to be distributed. Do not send forms directly to your current or former employers. The HR Office will distribute forms to previous employers and track their return. Instructions for completing the form are on the first page.

Mandated Reporter Training Act 126  (online- no cost)

In response to high profile child abuse cases, the Pennsylvania General Assembly made significant amendments to the Child Protective Services Law. All school employees, including contracted substitute teachers and independent contractors of school entities, must now receive Mandated Reporter training every five (5) years.This training can be done online through the Child Welfare Resource Center link: http://www.reportabusepa.pitt.edu/. You will need to register on this website, complete the online training, then print out and submit the Certificate of Completion (document must show your name and the date of training).
